Parents & Guardians of 7th Graders
Dear parents of incoming 7th graders,
Per the Massachusetts Department of Public Health kids entering 7th grade need to have Tdap and a second dose of Varicella prior to entering school. If your child has not had these shots please call your primary care physician to make an appointment to take care of this.
I will need a copy on file before the start of the 2014-15 school year.

Also, If your child plans to participate in a school sponsored sport in the fall he/she will need a current sports physical exam as well as a signed preparticipation form prior to starting practice. Forms can be dropped off at school during the summer between the hours of 8:00am and 12:00 noon Monday- Thursday.
